• Will not be disclosed to third parties without the consent of the client.

 

When disclosure can be made:

• Personal information can be disclosed where disclosure is necessary for the purpose of enabling another practitioner to provide services to the client and the client has consented to sharing of the information.

Access to records:

• Clients have the right to access their records held on paper or on computer. Access may be obtained by making a request in writing, and we will provide a copy within 45 days.
